SCHOOL FORMS

All Required Registration Forms (Due August 1st each year)

• DC Universal Child Health Certificate is valid for one year from “date of exam” noted on form. Must be updated annually. Please include dates and results for TB and Lead tests (required). A special note for returning students: a new form is due only when the form on file expires. Please check with the school office if necessary.

• DC Oral Health Required for all children 3 years of age and older. Certificate valid for one year from date of exam and must be updated annually. A special note for returning students: a new form is due only when the form on file expires. Please check with the school office if necessary.

• OSSE Licensing Forms Packet: This includes the Authorization for Emergency Medical Treatment Please the Registration Record for Child Receiving Care Away from Home and the Travel & Activity Authorization. Students will not be leaving the Nursery School campus for “Trips in the van/automobile” or “Field trips away from the facility,” so you may leave that area blank or cross it out. However, please check the box to allow your “child to play outside the fenced area.” Classes occasionally go for nature walks around the block or to view a visiting fire truck in the church parking lot, for instance. The District of Columbia requires this authorization form to be signed by a parent to allow us to walk your child to the parking lot or anywhere outside of the playground.
